Community Resource Team - 29.5 hours

We are looking for a part time (29.5 hours) Social Worker for the Vale Community Resource Service. The successful candidate would have the opportunity of working within Adult Services- Vale Community Resource Service (VCRS) and the Integrated Discharge Service (IDS). You would support the social work function of the multidisciplinary integrated health and social care reablement team and IDS.

VCRS is an integrated Health, Social Care and Third Sector Team based at Barry Hospital.

VCRS work with people in their own home to maximise functional independence in activities of daily living (ADL), thus reducing the need for admission into hospital and longer-term care and support services. It also offers therapeutic intervention and reablement support to people following an admission into hospital, so that they can return home as timely as possible.

The team includes: Occupational Therapists, Social Workers, Physiotherapists, Speech and Language Therapists, Dieticians, Nurses, Reablement Coordinating staff and Reablement Support Workers (Home Carers). The VCRS Social Workers work with individuals to establish ongoing support needs, linking in with care management teams within Social Services to set up ongoing packages of support if required.

The Integrated Discharge Service (IDS) work closely with Health and Age Connects to provide discharge planning support to citizens of the Vale of Glamorgan whilst they are in hospital. The Team is based at University Hospital Llandough (UHL) and consists of Social Workers and Accommodation Discharge Co-ordinators who work in partnership with health professionals to assist in planning and arranging the safe discharge of adults from the Princess of Wales Hospital, University Hospital of Wales (St David’s Hospital Cardiff), University Hospital Llandough and Barry Hospital.

The Social Work role would require you to use WCCIS as the local authority case management system, care co-ordinate, complete well-being assessments, care planning and review.

Required –

Social Work Degree (or equivalent)

Social Care Wales Registration

Enhanced DBS on Update Service (can help with this if needed)
