Due to the expansion on the business Harbour Healthcare are recruiting for an Intermediate Care Coordinator with this brand-new unit at the beautiful Gotton Manor in Taunton TA2.

Job Purpose:

  • To provide leadership to the care team within the service
  • To provide care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations, under the direction of the Manager
  • To supervise the care services within the organisation in accordance with agreed standards
  • To maintain skills at a current level, and undertake Ensure service users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing
  • To supervise team and ensure that all team contribute to the efficient running of the service
  • Be responsible for promoting and protecting the welfare of those individuals supported by the service
  • Completion of allocated audits in a clear and concise manner
  • To meet and greet new prospective residents and their family members.
  • Complete supervisions quarterly with the nursing and Care team as allocated by your manager
  • To work with the social worker/discharge liaison/hub relating to admissions and discharges of residents.
  • Coordinating safe admissions to the service.
  • Communicating with associated relatives / NOK / POA for each new admission.
  • Compiling the care plan and associated risk assessments of new and existing residents to The Coach House.
  • Working closely with the OT, Physiotherapists and enablement team on supporting each resident and achieving the resident's desires outcomes.
  • Coaching and supporting the care and nurse team on correct delivery of care for each resident.
  • Supporting the smooth transition of each resident once their package of reablement has completed.
  • Ensuring each resident has access to facilities required to aid in their reablement.
  • Preparing the discharge for each resident.
  • On call support (rota basis).

Please note that all our positions require an Enhanced DBS check in relation to Children and Adults.
